REPORTING INJURIES
IF YOU ARE INJURED ON THE JOB ....
- Notify your supervisor at the job site ... Immediately
- Call, or have your supervisor at the job site call BesTemps
- Follow the exact instructions given to you by the staff at BesTemps
Unless you are in a life-threatening situation, do not seek medical attention without clearing it with BesTemps first. A drug & alcohol test will be administered at the time of medical treatment. I understand and agree that the failure of a drug or alcohol test is cause for the cost of the test to be passed on to me.
* You must document the fact of your injury as all claims will be investigated.
FAILURE TO FOLLOW THESE INSTRUCTIONS MAY JEOPARDIZE YOUR RIGHT TO A CLAIM.
If you file a false worker's compensation claim, you will be prosecuted. I have read the above policy regarding workers compensation and job-related injuries and understand I must notify my BesTemps office immediately of any job-related injury.
SAFETY
I agree to follow all "safety policies" and to ask a supervisor at BesTemps or at the host company I may be assigned to, if I am unsure of how to handle any chemical. I also understand that the material safety data sheet, outlining all substances I may be exposed to at the host company, is available for review upon my request.
UNEMPLOYMENT
I will notify you when my temporary assignment ends. If I don't, it means that I am not available for work. I understand that my employment may be terminated at any time for any reason, including but not limited to, the failure to comply with instructions of supervisors or policies of the companies I am assigned to, incompetent on assignments, failure to show up for or complete an assignment; and/or the use of, possession of, or being under the influence of illegal drugs or alcohol will result in immediate termination of employment
I understand that, if I am sent on an assignment, I am expected to report to work every day that I am scheduled and on time. You have the right to refuse any job. However, if you accept a position we offer you and then do not report to work, or do not seek work daily after each assignment, it will be reported to the unemployment office, which could result in a denial of unemployment benefits. If you do not want a particular assignment, please do not accept it, and wait for another assignment.
I understand that if I am sent on an assignment and I will be late or have to miss work for any reason, I must contact a staffing coordinator of BesTemps and the company I am assigned to as soon as possible.
GROUNDS FOR TERMINATION
If I do not report to work and fail to notify BesTemps of my absence or if I walk off the job, this would be considered willful misconduct and would be grounds for termination. I understand that failure to follow these standards will be cause for termination. I also understand that padding a timecard or forging an authorized signature on a timecard will result in legal action and immediate termination of my employment with BesTemps.
My employment may be terminated by you at any time, for any reason, and you will only be liable to me for wages earned up to termination.
AGREEMENT
For good and valuable considerations, the parties hereto agree that the employee shall release and hold forever harmless that company from and against any and all responsibility and liability for bodily injury or property damage arising out of the operation of vehicles provided by the employee in connection with the work to be performed for the company by such employee.
Confidentiality Agreement
Any employee of BesTemps, must keep confidential all information learned from or about our clients. This includes, but is not limited to:
- No pictures or videotaping can be taken while on assignment or on the property of any company/client.
- No posts or comments on any social media in regard to our policies, clients, supervisors, or co-workers.
Any violation of this policy could result in immediate termination.
CONSENT FOR RELEASE OF INFORMATION
I authorize BesTemps to make inquiries of all my past employers, educational institutions and references concerning my prior employment, the verification of my educational background, and personal character. I further authorize all past employers, educational institutions and all other individuals providing references to respond to verbal and written inquiries from BesTemps regarding my past employment, the verification of my educational background, and personal character. I authorize BesTemps to release background check results and drug testing results to their clients upon request. I hereby release all such persons from any liability and damages incurred as a result of furnishing this information.
If I am hurt or injured during my employment with BesTemps, I do hereby consent to the recipient of this authorization to disclose, furnish copy of, or permit bearer to view, copy record or release to BesTemps all medical records relating to me, either past or present, physical or mental, including but not limited to all histories, examinations and test results, billing and payment schedules, diagnostics and progress notes, prescriptions and other orders, admission and discharge summaries, X-rays and other photographs.
Harassment & Workplace Violence
BesTemps is committed to preventing harassment and violence and maintaining a safe work environment. All employees, customers, vendors and business associates should be treated with courtesy and respect at all times. Conduct that discriminates, harasses, threatens, intimidates, or coerces another employee, customer, vendor or business associate will not be tolerated. This includes all BesTemps offices, and property, as well as all BesTemps client’s offices, property, and worksites.
Indirect or direct threats of violence, incidents of actual violence and suspicious individuals or activities should be reported as soon as possible to a supervisor, security personnel, human resources (HR), or senior management.
BesTemps will promptly and thoroughly investigate all reports of threats of violence or incidents of actual violence. Anyone found to be responsible for threats of violence, actual violence or other conduct that is in violation of these guidelines, will be subject to prompt disciplinary action up to and including termination of employment and criminal charges.
Permanent Position with a BesTemps client
If I work for you, I will be your employee. I will obtain permission from BesTemps, Blue Hen Staffing, Diamond State Staffing, before discussing permanent employment with your clients. I understand that if I am offered a permanent position with a BesTemps or Diamond State Staffing client, whether it be before the required 12 weeks or 480 hours, or after, I am not obligated to accept the position.
I also understand that if I decline a permanent position, I still have the right to work for BesTemps and be offered future assignments. My decision to decline a permanent position will in no way affect my employment or professional standing with BesTemps.
If I choose to accept a permanent position with a BesTemps client, I understand that I have voluntary quit BesTemps for other employment. The BesTemps/Blue Hen Staffing Inc./Diamond State Staffing Inc. client will become my employer at that time.
